Physical Therapy

PT Initial Assessment (60 mins): This visit is required of all new PT patients and will include: a brief intake to discuss current issues, procedures to assess current status, appropriate treatment for that day, and a discussion to create a plan for moving forward.  After this initial visit, your subsequent visits can be booked as "PT Treatment Visit".  

Cost: $180*

PT Re-Evaluation (60 mins): A visit dedicated to assessing patient progress by performing tests to measure current status, updating goals, and reviewing the PT plan of care. If you have been a patient of Centreboard in the past and are resuming PT for the same condition after a hiatus of care, this visit is for you.

Cost: $160*

PT Treatment Visit (30-40 mins): This is a treatment visit.  After the initial PT assessment, subsequent sessions may include and are not limited to: manual therapy including soft tissue massage, joint mobilizations, myofascial release and stretching; therapeutic exercise recommendations; use of modalities such as heat, ice, cupping, and intermittent compression; and provision of recommended follow-up rehabilitation.  Visit time will depend on individual patient needs and plan of care discussion between patient and provider.

Cost: $140*

*Cost listed is the rate for cash-based patients (for those NOT using insurance). If you choose to use your insurance to cover your services, the cost of your visits will be subject to your individual in-network insurance coverage and benefits (deductible, coinsurance, copay). It is the patient’s responsibility to be informed of their PT coverage and benefits prior to appointments, including whether or not Centreboard is in-network or out-of-network for your plan and if virtual/telehealth is a covered service for your plan when applicable.

Payment: Centreboard is currently accepting several major insurance policies, including: Anthem BCBS, Harvard Pilgrim, Cigna, Aetna, United Healthcare and Worker’s Comp. Centreboard can submit claims for you to any in-network insurance plans.

Centreboard is an Out-of-Network provider with all other insurance carriers (and some specialty plans within the above insurance carriers). If we are not currently in-network with your insurance, that does not mean an insurer won’t pay for the services. Upon request, Centreboard can provide you with a superbill which will include the NPI (National Provider Identifier) number, appropriate ICD-10 (International Classification of Diseases) diagnosis codes as well as CPT (Current Procedural Terminology) billing codes. You may choose to submit this to your insurance carrier for reimbursement based on your plan benefits.

It is imperative for you to understand your outpatient Physical Therapy insurance benefits before deciding if you would like to use your insurance. Additional considerations include and are not limited to: whether or not your insurance requires a preauthorization or a physician’s referral, whether or not there is a copay or a deductible/other out-of-pocket amount due before services are covered, and at what percentage services are covered.

Massage Therapy

Massage: Bodywork session to be tailored to the client’s individual needs as discussed between client and therapist.  May include: stretching, Swedish or deep-tissue massage, trigger point release, myofascial release, or instrument-assisted tissue mobilization. Cupping can be added on for $10. New prices effective 10/1/2023.

30 minutes: $65

45 minutes: $85

60 minutes: $110

Cupping: Cupping therapy can be described as myofascial decompression and is a modern adaptation of the ancient art. While massage is performed by applying pressure, cupping instead involves use of cups to create decompression (“negative pressure”, suction) to pull, lift, open and expand the body’s soft tissues, targeting the fascial layer. There is also a component to cupping that involves “activating” nerves in an area to let the body respond to the activity in a non-painful way. The mechanism is through moving peripheral fascia and/or nerve gliding, creating a cascade of events.  The goal of cupping is to increase tissue space to decrease pain and restore movement. Cupping can help to relieve muscle tension, decrease congestion and swelling, improve overall blood flow and promote cell repair.  Cupping marks (which can be areas of discoloration that look like bruises) can be a response to cupping, as well as redness and itching skin, due to the histamine-type response that can occur. Cupping treatment sessions will primarily involve a variety of static and dynamic cupping techniques, with or without active or passive movement, with less emphasis on massage application.

15 minutes (1 body area): $35

30 minutes (1-3 body areas): $65

45 minutes (2-4 body areas): $85

Intermittent Compression Therapy: Rapid Reboot leg sleeves use a technology involving air pressure to provide dynamic pneumatic compression to improve circulation. Compression therapy uses controlled pressure to increase blood flow in your legs and improve blood flow to the heart. At the same time, it supports your veins and decreases swelling. This may help speed up muscle soreness and injury recovery which can help to improve performance. Pressure options and cycles of compression individualized to your needs and comfort and treatment time options are 10, 20 or 30 mins.  This service can be added pre or post massage OR can be used at a stand-alone treatment and can be done before or after a workout.

10 minutes: $15
20 minutes: $25

$100 per hour (2 hour minimum)

CANCELLATION POLICY: Your appointment time is reserved just for you, one on one, with your provider. A late cancellation or missed visit leaves a hole in the therapist’s day that could have been filled by another patient. As such, we require 24 hours notice for any cancellations. Patients who provide less than 24 hours notice confirming cancelling the appointment will be charged a $40 cancellation fee to the card on file. Each patient/client will be given one “freebie”, because we understand unforeseen circumstances can affect your ability to keep your appointment. If you are undecided about whether or not you will cancel your appointment, you must leave more than 24 hours to communicate with your provider to make the decision, understanding that your therapist may not be able to communicate immediately when you reach out. The 24 hour threshold is based on the time you officially cancel the appointment and is not based on when you reach out with uncertainty about keeping or cancelling your appointment. You will be reminded of the policy on the first occasion of a late cancellation to make sure there is no misunderstanding about the policy. A no show visit will be charged the entire visit rate. If you are late for a session, you may lose some of that session time. Thank you for understanding and honoring the time you have set aside for yourself.

COVID-19 Protocols

Centreboard’s protocol, in accordance with the US CDC recommendations, is updated as followed, effective May 24, 2021:

Fully vaccinated persons are not required to wear face coverings and those who are not fully vaccinted are recommended to continue to employ face coverings during sessions. All patients/clients will continue to be screened for Covid 19 symptoms prior to visit.  Please continue to utilize hand hygeine strategies, with available hand sanitizer upon entering or soap and water at the sink.

Your provider will continue to disinfect the room/surfaces between every client and provide the most ventilation possible with a combination of opened doors and windows either before/during/after each treatment, and air purifiers in treatment room operating during business hours.
