The Insurance Myth: Why Quality Care Doesn’t Have to Break the Bank
Navigating the “Dueling Voices” of Physical Therapy to Find What Works for You
If you’ve spent any time on social media lately looking for fitness or rehab advice, you’ve probably seen the trend. There’s a growing movement of “cash-pay” physical therapists—specialists who don’t take insurance at all—telling the world that if you want the “good stuff,” you have to pay out of pocket. The narrative is often that insurance-based clinics are “mills” where quality is sacrificed for volume.
As the owner of Centreboard, a DPT (Doctor of Physical Therapy), and an LMT (Licensed Massage Therapist), I’m here to tell you: It doesn’t have to be an either-or.
When I opened Centreboard five and a half years ago, I made a very intentional choice to accept major insurance. Why? Because I live here in Maine, I’m raising my family here, and I know that most of us are already paying hundreds of dollars a month in premiums. I personally can’t justify spending $280 for a single visit when I’m already paying for a benefit I’m not using.
I want quality care to be accessible. I want you to be able to use the benefits you’re already paying for without feeling like you’re getting “PT-lite.”
Quality vs. Coverage: The Real Difference
There is a misconception that because an insurance company dictates what we can bill for, they also dictate the quality of the soul we put into the work. That’s simply not true.
The main difference is often the “goal post.”
Insurance cares about “medical necessity.” They want to make sure you can get in and out of your car, walk up your stairs, and live your daily life without pain.
Specialized Cash-Pay often focuses on high-level performance—like shaving two minutes off your marathon time.
But here’s the secret: We can do both. If you sprain your ankle, your insurance will likely cover the “basic” PT to get you back to walking and light jogging. That $25 or $30 copay is a lot easier to swallow than a $200 out-of-pocket fee. Once we’ve met those medical goals, if you want to see a running-specific coach for elite performance, we’ve at least saved you the cost of the initial rehab.
Understanding Your Benefits (Without the Headache)
I know the word “deductible” makes most people want to close their eyes and hope for the best. Most people know they have insurance, but they have no idea what it actually covers for rehab.
Variable coverage is the norm, not the exception. Even within a single company like Anthem, one person might have 10 visits and another might have 120. One might have a $20 copay, while another has an $8,000 deductible to meet first.
Here is the “Pro Tip” for navigating this:
Flip your card over. Call the number on the back.
Ask for “In-Network, Outpatient Physical Therapy benefits.”
Ask three questions: Do I have a copay? Do I have a deductible? Do I need a referral from my doctor?
At Centreboard, we are currently in-network with Medicare, Anthem BCBS, Harvard Pilgrim, Cigna, Aetna, United Healthcare, Maine Community Health Options, Taro, and Worker’s Comp. If we aren’t in-network for you, we can still provide a “superbill” for you to submit for potential reimbursement.
Why We Still Jump Through the Hoops
To be honest, it’s a lot of work for us. Accepting insurance means extra paperwork, justifying medical necessity, and dealng with reimbursement rates that haven’t significantly increased in twenty years. That’s why many therapists are leaving the insurance world behind—it’s a tough cycle.
But at the end of the day, my goal is to help you find stability and lateral resistance (just like the centreboard of a boat!). Whether you use insurance or choose our cash-pay rates because your deductible is too high, the treatment you get in my office is the same. It’s hands-on, it’s movement-based, and it’s focused on you.
Don’t let a social media trend or a confusing policy stop you from feeling better. Your body deserves the care, and your wallet deserves to use the benefits you’ve earned.
